Protecting residents from abuse and neglect isn’t just a compliance issue, it’s a moral imperative. Every assisted living community must foster a culture where safety, dignity, and accountability are non-negotiable. Unfortunately, recent investigations in Ohio have revealed troubling lapses in how some facilities respond to abuse allegations, manage staff conduct, and protect vulnerable residents.

In one case, a cognitively impaired resident was sexually assaulted by another resident. The caregiver who received the report failed to notify anyone immediately, allowing the resident to return to bed without intervention. The abuse wasn’t reported until six hours later, delaying medical care and law enforcement involvement. A SANE (Sexual Assault Nurse Examiner) later confirmed physical evidence of assault.
Another incident involved a fully dependent resident who was left without incontinence care or repositioning for an entire 12-hour shift. She was found face down in a pool of blood after falling from her Broda chair, suffering a brain injury and scalp laceration. Staff cited confusion over care responsibilities, an unacceptable breakdown in communication and accountability.

These cases underscore the need for:
Immediate reporting protocols that trigger swift investigation and protective measures.
Staff education on recognizing abuse, psychosocial symptoms, and burnout.
Competency checks and background screenings, including sex offender registries and licensing verifications.
Resident assessments to determine capacity for consent and identify behavioral risks.
Emergency discharge planning when safety cannot be guaranteed.

Facilities that responded effectively implemented corrective actions such as enhanced supervision, staff separation, policy revisions, and ongoing training. Resident abuse prevention must be embedded in every layer of operations, from hiring and onboarding to daily care and incident response.
