From Volatility to 5-Star Performance: How MDS Management and Therapy Services Changed Outcomes Across Five Skilled Nursing Facilities


Quality Measure performance feels increasingly difficult to control. Claims-based methodologies continue to evolve. Staffing turnover affects consistency. One quarter may show improvement while the next introduces new concerns. In competitive referral markets, even short-term declines can impact reputation, referrals, and reimbursement.


For one ownership group operating five skilled nursing facilities, performance had become inconsistent across the portfolio. Some buildings carried decline risk. Others struggled with quarter-to-quarter volatility. Leadership recognized they needed more than periodic audits or reactive interventions. They needed structure, visibility, and a stronger operational approach to MDS management.


That shift became the foundation for significant results.


Repositioning MDS From Compliance to Strategy


The organization partnered with Engage Consulting to implement an embedded MDS Management model focused on accountability, operational execution, and measurable performance improvement. 


Rather than treating MDS as a documentation function, the model positioned it as a driver of clinical outcomes, reimbursement accuracy, and Quality Measure performance.


The strategy included: 

  • Weekly facility-level MDS working sessions  

  • Real-time Quality Measure tracking and trend analysis  

  • Coding optimization and PDPM validation  

  • Interdisciplinary education for MDS, nursing, and leadership teams  

  • Ongoing consultant oversight and auditing  


The objective was not simply to improve scores for one reporting cycle. The goal was to build systems capable of sustaining performance long term. 

 

The Results 

The impact across the portfolio was substantial. 

  • 5 of 5 facilities achieved 5-Star Overall QM ratings  

  • Several buildings sustained 5-Star performance for two consecutive years  

  • Medicaid Quality Incentive Program gains ranged from 12 to more than 21 points  

  • Medicaid reimbursement improvement reached as high as $24.23 per point per day  

  • PDPM coding accuracy improved to between 97% and 100%  


Just as important, previously volatile buildings became more stable and proactive in their performance management approach. 


Portfolio-wide, 5-Star QM performance strengthened referral confidence and marketability while strong audit performance reduced compliance exposure and supported cleaner claims submission. 


Expanding the Partnership Into Therapy Services 

Based on the success of the MDS Management partnership, ownership expanded the relationship to include full contract therapy services through Concept Rehab


Leadership wanted to ensure the transition protected staffing continuity, resident care, billing workflows, and day-to-day operations. 


The implementation process focused heavily on communication, planning, and operational coordination across each building. Once fully implemented, the partnership expanded into broader therapy and quality improvement initiatives. 


Connecting Rehabilitation to Quality Outcomes 

Using the Integrate Advantage therapy model, Concept Rehab aligned rehabilitation services directly with portfolio quality goals through: 

  • Targeted therapy interventions tied to QM performance gaps  

  • Intensive Section GG education and coding accuracy training  

  • Standardized assessment and treatment plan alignment  

  • Collaborative hospital readmission reduction initiatives  


The result was stronger alignment between rehabilitation delivery, interdisciplinary collaboration, reimbursement accuracy, and Quality Measure performance. 


Sustained Success Requires Structure 

Many organizations can produce temporary gains. Sustaining performance across multiple buildings requires operational discipline, accountability, consistent execution, and often, a strong partnership. That became the defining outcome of this partnership. 


The buildings did not simply improve star ratings. They built systems capable of maintaining performance, reducing volatility, and strengthening long-term outcomes across the portfolio. This partnership demonstrates that sustainable improvement in skilled nursing quality measures is achievable when strategy, execution, and accountability are fully aligned.
